How to Refer a Client
We’ve made the referral process easy for healthcare providers, educators, and community organizations:
- Complete the Referral Form
- Provide the client’s basic demographic and contact information.
- Include details about the reason for referral, any known diagnoses, and the services they may need.
- Submit the Referral
- Email the completed form to info@canyonhealthcenters.com
- Fax it to 480-903-7171
- Call us directly at 480-903-6463 to discuss the referral if you need assistance.
- What Happens Next
- Our team will review the referral within 1-2 business days.
- We’ll contact the family to discuss their needs, schedule an assessment, and explain the next steps.

Eligibility Requirements
